Sebagai demonstrasi kegunaan klinis, para peneliti bertujuan untuk memvalidasi kembaran digital ini untuk evaluasi praklinis mengenai keamanan dan kemanjuran terapi yang ditargetkan pada EVLP.<\/p>\n<h2 class=\"pb-4 pt-2 text-2xl\"><strong>Bagaimana model paru-paru digital dikembangkan dan dilatih?<\/strong><\/h2>\n<p class=\"\">Upaya pemodelan didasarkan pada kumpulan data klinis komprehensif yang berasal dari kasus klinis EVLP yang dilakukan antara tahun 2008 dan 2024 di Rumah Sakit Umum Toronto. Data multi-modal yang dihasilkan mencakup spektrum luas karakteristik paru-paru manusia, seperti pengukuran fisiologis, fitur pencitraan medis, transkriptomik, dan berbagai biomarker proteomik, metabolomik, dan biokimia.<\/p>\n<p class=\"\">Untuk membuat model perkiraan, para peneliti melatih model multi-modal menggunakan k-fold cross-validation, menggunakan model berbasis pembelajaran mendalam yang dikenal sebagai Gated Recurrent Unit bersama dengan model XGBoost berbasis pohon. Kinerja model diukur secara kuantitatif menggunakan rata-rata kesalahan absolut dan persentase kesalahan absolut.<\/p>\n<p class=\"\">Dua strategi pemodelan berbeda diterapkan dalam mengembangkan kembaran digital paru-paru manusia. Strategi digital twin &#39;statis&#39; menggunakan satu set data dasar statis fungsi paru-paru sebagai masukan untuk memperkirakan fungsi paru-paru di masa depan. Sebaliknya, pendekatan digital twin &#39;dinamis&#39; dirancang untuk dikalibrasi ulang menggunakan data fungsional tambahan, dan hanya memperkirakan titik waktu berikutnya berdasarkan data fungsi paru-paru terbaru.<\/p>\n<p class=\"\">Untuk membuktikan validasi konsep paru-paru digital ini, para peneliti mempelajari kohort dari 16 paru-paru yang sebelumnya telah menerima aktivator plasminogen tipe jaringan (tPA), sebuah terapi trombolitik. Keuntungan utamanya adalah kemampuan si kembar digital untuk berhasil memprediksi hasil kontrafaktual\u2014keadaan paru-paru jika terapi tidak diterapkan. Kemampuan ini memungkinkan analisis statistik berpasangan antara paru-paru yang diberi pengobatan dan kembaran digitalnya yang tidak diobati. Sebagai contoh yang representatif, satu kasus EVLP yang diobati dengan tPA menunjukkan penurunan PAP dibandingkan dengan kembaran digitalnya, tanpa peningkatan edema yang signifikan.<\/p>\n<p class=\"\">Saat melakukan analisis berpasangan pada kohort paru-paru manusia yang diobati dengan tPA, metodologi digital twin menunjukkan penurunan PAP yang signifikan dalam waktu dua jam setelah pengobatan (<em>P<\/em>=0,031). Temuan ini sangat kontras dengan hasil desain studi dua kelompok konvensional, yang bila diterapkan pada data kohort yang sama, tidak menunjukkan perbedaan signifikan pada PAP pada satu atau dua jam pasca perawatan. Akibatnya, penelitian ini menunjukkan bahwa hanya pendekatan digital twin yang cukup sensitif untuk mengidentifikasi penurunan PAP yang disebabkan oleh pengobatan praklinis.<\/p>\n<p class=\"\">Pendekatan kembar digital menghasilkan model virtual paru-paru manusia yang dibangun berdasarkan kumpulan data klinis terbesar yang tersedia di dunia <em>ex vivo<\/em> prosedur perfusi paru-paru, negara bagian Zhou dan tim.
